《 Declaration Form to HKSAR 》for Macau SAR Permanent Resident

重要事项 IMPORTANT NOTE	所有护照必须于出发日期计起，最少有效６个月。儿童之旅游证明如依附在父母亲之护照／回港证，除非由该名监护人陪同下，否则，不准单独旅游。如有乘客因未能遵守上述出入境规例而导致未能离境者，所缴之费用概不退还。	All valid Passports must have at least 6 months validity from the actual date of sailing. For a child whose traveling document is attached to the parent’s passport / HK Re-entry Permit, he or she is not allowed to travel independently without the accompanying parent. Strictly NO REFUND will be made when passengers are denied embarkation for non-compliance of the above immigration requirements.
乘客限制 PASSAGE RESTRICTIONS	怀孕二十四星期或以上之孕妇不能登船航游。丽星邮轮可视乎乘客的健康状况，保留对未能于报名时提供健康情况的乘客拒绝登船之权利，并无须对该名乘客已缴付的费用作任何赔偿。 18岁以下的乘客必须与随行之18岁或以上的乘客同房，或住宿于相连客房。而该18岁或以上的乘客亦须同意于整个航行中负责照顾该名18岁以下的乘客。	Passage for pregnant woman will only be permitted before her 24th weeks of pregnancy. Star Cruises reserves the right to refuse to embark any passenger whose condition is made known prior to sailing and shall not be required to refund any portion of the fare paid by such passenger. A passenger under 18 years of age should be accompanied in the same or connecting stateroom by a passenger 18 years or older who expressly agrees to be responsible for the under 18 passenger throughout the cruise.
登船时间 BOARDING TIME	请留意关闸时间！所有乘客必须于关闸前办理手续登船。乘客如因迟到而未能登船，均当自动放弃论，所缴费用概不退还。	Please note the respective gate close time! All passengers are expected to check in and board ship before gate closes. Passengers who are denied boarding due to late check-in/ boarding will be treated as no-show and no refund will be granted.
